This is the website of the EU funded transnational project PERCENTAGE (PERson CENtred Training: AGE care planning

The Percentage project set out to develop a training tool which would enable those working in the care and health occupational sectors to produce high quality and appropriate care-plans for older clients.
A partnership of 6 organisations drawn from 5 European countries and representing the public, private and voluntary sector working together over 26 months in total to achieve this. We hope that our learning programme will be something new; it will be based on a person-centred approach to care giving; it will develop skills in new ways and will enable those working directly with older people to gain a new form of recognition for the skills and knowledge they use in their everyday work.
The project takes the philosophy of person-centred interventions as its starting point in an attempt to successfully provide a vocational development tool capable of addressing serious and growing issues around elderly care and parallel demographic factors.
Percentage is designed to equip frontline care workers with the ability to work with their service users and their families: to explore their care needs; to devise and evaluate options for delivery care; and to design, implement and evaluate a care plan.
Care plans are often devised by supervisory staff or care managers. However, it is the frontline staff who have the closest contact with each service user. They know them well and they can understand their needs. They are also well placed to know what care tasks are needed and how best to deliver them. The Percentage Programme helps frontline care workers translate this knowledge into devising care plans.
